Advocate-Assisted binder customers may provide healthcare and other sensitive personal information so PMCV can organize and enter that information into the purchased binder.
Only provide information about yourself or another person when you have permission or legal authority to do so.
PMCV will provide instructions for submitting sensitive information through the designated method. Standard website forms, social-media messages, text messages, and ordinary email may not be appropriate for transmitting detailed healthcare information. Do not send sensitive information through an unsecured channel unless specifically instructed.
Information provided for a binder is used to complete the requested product and related customer service. It is not used to make medical decisions, diagnose conditions, recommend treatment, or replace advice from licensed professionals.
