About Healthcare Surrogate Services in Temple Terrace
-
A professional healthcare surrogate is someone you legally designate to make healthcare decisions within the authority you grant. We take time to understand your wishes, values, and priorities so they can guide decisions if you cannot speak for yourself.ription text goes here

This service may be a good fit if you live alone, have loved ones far away, lack someone available to serve, or prefer a professional in this role. Planning ahead creates time to build a relationship before support is needed.
-
We begin by discussing your healthcare wishes, important contacts, and existing planning documents. We organize the information relevant to our role and establish a communication plan. When legally authorized, active services may include communicating with healthcare providers, participating in care discussions, and making healthcare decisions guided by your wishes. Specific services and fees are outlined in your agreement.
-
Initial setup: $495.
Annual renewal: $495, beginning 12 months after your agreement’s effective date.
Active professional services: $175 per hour, billed in 15-minute increments for remote work, with a one-hour minimum for in-person services.
Travel and mileage charges may apply. Setup and renewal fees are separate from active service charges. We review the terms with you before services begin.
-
Start with a 15-minute consultation to discuss your needs and whether we are a good fit. If we move forward, we complete a suitability and conflict review, discuss fees, and establish a service agreement. A separate, valid legal designation is required to appoint your healthcare surrogate. We coordinate with your attorney as needed; we do not provide legal advice or prepare legal documents.
-
Based in Bradenton, we offer professional healthcare surrogacy across West Central and Southwest Florida, with in-person support by arrangement. Remote options may also be available nationwide. Contact us to discuss your location, applicable state requirements, and whether we can meet your needs.
-
Authority depends on your legal designation and applicable law, including when that authority begins. Signing a service agreement does not appoint us as your surrogate. While you can make your own healthcare decisions, your decisions control. This service does not grant financial power of attorney or authority to manage your money.
-
This is not a 24/7 or emergency-response service. We establish contact procedures and availability expectations during enrollment. For a medical emergency, call 911. Healthcare providers should use the contact information in your planning documents when surrogate involvement is needed.
