The binder includes sections for health history, medications, providers, insurance, appointments, tests and results, referrals and follow-through, hospital visits, discharge planning, important documents, and notes.

• A completed digital fillable PDF copy
• A $300 prepaid professional-time deposit, covering up to two hours of Advocate-Assisted organization at $150 per hour

Peace of Mind Care Visits will organize and enter the healthcare information you provide. Professional time is tracked and billed in 15-minute increments.

If your binder requires more than two hours to complete, you will be notified before any additional work is performed. Additional time is billed separately at $150 per hour. No additional work or charges will occur without your approval.

If less than two hours is required, any unused portion of the $300 professional-time deposit will be refunded after your binder is completed. The $59 printed binder charge is nonrefundable once printing and assembly have begun.

Shipping or local delivery is charged separately at checkout.

HOW IT WORKS: After your purchase, Peace of Mind Care Visits will contact you with instructions for securely providing the information needed to complete your binder. You are responsible for reviewing the information for accuracy and notifying us of any corrections before the final binder is printed.

IMPORTANT: We organize and enter the information you provide. This purchase does not include obtaining medical records, contacting healthcare providers, researching missing information, interpreting medical information, or providing ongoing patient advocacy services. Additional advocacy or healthcare navigation assistance may be purchased separately if needed.

MADE TO ORDER: Each Advocate-Assisted binder is individually completed, printed, assembled, and organized after the necessary information has been received. Your completion timeline begins once all requested information has been provided and confirmed. Shipping charges and delivery time are additional.

The completed PDF and printed materials are licensed for personal use by one individual. They may not be shared as blank templates, copied for multiple people, resold, or used commercially.
